Castiel notices that Dean calls Sam by many names humans generally consider to be offensive.

Dean calls the younger man "bitch" and "geek boy" and "Fabio". (Castiel doesn't understand that last one, but he notices Sam grimaces, so he assumes it's a derogatory expression.)

He also notices Sam responds to these titles in the likeness of his brother.

Sam tosses back retorts like "jerk" and "slob" and "Abercrombie and Fitch face". (Another name Castiel doesn't fully comprehend but assumes is bad in some way.)

What Castiel does understand is that none of these remarks seem to bother the brothers. Not when they come from one or the other. It's like a language all their own. Their code of endearment showing they accept each other.

It is with this knowledge in mind that Castiel smiles the first time Dean and Sam refer to him as "nerd angel".
