I really like the story, yours is the first I've seen with Luffy given the Operation Fruit. Great story overall and I'm curious about the pairing.

Dude this is a great first chapter. Come back and write more if it. I just Wanna see what happens next and how different the adventure would be.

This looks spectacular. While I am a bit concerned for Law, taking a showing of Sarie who we have yet to learn more of, Luffy with a different Devil Fruit, and Luffy actually taming the Lord of the Coast were each a treat. A Pirate King who is also the World's Greatest Doctor and Fighter is awesome!

However, is it possible for you to include Law's presence in the story? I think one way he could've survived his sickness, is that Corazon took him to either Vegapunk or the Germa Kingdom and paid for a suspended animation thing that could've kept Law alive, even if he's still a little kid, but I'll let you be the judge of that.

As for the pairing, well, I'm a fan of possible harems that can work per story, and lately I've figured that 12 ladies would be quite enough. There are a lot of possibilities after all. With his Op-Op Fruit, Luffy can remove Nami's Arlong tattoo as well as the slave markings on people like the Boa sisters without causing her too much injury, I liked Huntsman8888's reason for Robin as an option, and there are some girls that Luffy could be paired with that aren't listed or done much of, like Perona, Viola, or someone I can't think of right now.

Bottom line, I like this story, and I hope you continue with your good work whether it's the stories you've already written, will think of, or take up.

Robin for the pairing, all the extra hands to help with surgery, not to mention how she treats Chopper so it would be a good idea to have Luffy and Chopper working together wit Robin using the Hana Hana no mi to pass them supplies.
