If the "red water" is what is eventually compressed into a Stone, then does that mean Edna might be dealing with a bunch of souls poking at her due to the dousing? Given Angel's presence, I doubt they could do as much as without her shielding/anchoring, but the differences in the Elrics' studies means she would also be a lot more aware of any such effect than canon!Ed would have been. A large amount of being out cold is from the exhaustion and injuries, but something like needing to corral/subdue/free a myriad of trapped souls now stuck to her from the water would drag out her recovery, as would simply needing to adjust to the sudden dramatic difference in what she is sensing around her. The sensation of trapped souls stuck to her physical form would be quite different to the crawling sensation underground they are used to, even if both that sensation and regularly carrying that Stone in a locket could be considered effectively training for this moment thus making it more likely she wouldn't be driven insane by it.
We've seen nominally-sane-ish characters with actual or pseudo-Stones absorbed into themselves, but how many of them had anywhere near the sensitivity to the souls within that Edna and Angel do? Scar was aware, at least knew well what it was regardless of his ability to sense them as more than energy, but his were contained pretty strictly to his arm even if he could use the energy fluidly. The Homunculi are all batshit in some way and only exist so stably because of said stones. Hoenheim is ancient and also not-quite-sane, just being that old would do that and his life/lives? He might be better than the Homunculi, generally, and better at functioning as a "normal" person to average detection, but he still doesn't quite count as completely sane and he /has/ had the longest amount of time to adjust to such senses if he also counts as someone similarly sensitive as Edna. He may have developed that sense more gradually, though, from sheer exposure over the centuries, making it less of a severe shock to the system than Ed got.

Maybe Al wakes up in a few days, miserable but recovering, but for some reason no matte how well she is healing physically (maybe a little too well, to the others' eyes considering the worries about the "red water", though that may or may not actually be Angel and Magic's fault) she isn't waking up as well. Stuck in a hospital bed is certainly one time recognition of her actual gender would come up, especially as she is out cold and can't do anything to try to fuzz it. If she's healing just a little too well but not waking, it could partly be because the exhaustion is so severe that reflexively healing herself as a passive magic thing is draining the tiniest bit of magic that is recovering, dramatically slowing the magic-regen even further so she literally hasn't got the energy to also wake up yet. Al could probably mask some of that by pointing to the little they've told about Alkahestry, except he's also pretty alarmingly exhausted and will at minimum get yelled at for "healing his sister when he himself hasn't the reserves to spare, stuck in the other bed in their hospital room". I'm pretty sure he'd rather fake boosting her healing rate and all that would entail than let anyone get too curious about how/why Edna's natural healing rate is enhanced even minorly, given all they know. Maybe they'd trust some of their closest colleagues, particularly Hughes, Hawkeye, and Mustang, but under the circumstances there are too many others who could and would be aware and paying attention to /not/ mask the full extent of things for their own safety. It's not like they haven't talked about Alkahestry and Al learning to heal with alchemy while they were in Xing, after all, so at least that's "old news" just some of the first in-person witnessing of it by the military. They could even claim there is some kind of technique or training side-effect that can boost one's natural rate of healing, akin to healing trances except it would have to be a passive side-effect of their training for Edna to be using it while unconscious or that would also be bringing Attention they don't want, if not as severely as "red water assisted regeneration". Combining the two would help explain why Edna was healing a hair too fast if not waking up while also covering the interest in how Al can manage helping her heal when in such bad shape himself. Being able to manage something like that does sound like something Al would do, but lessening the impressive-ness slightly also lessens the Concerning edge to the interest of others which is Of The Good. And the suspicion of anyone who is considering how much of the story might be bullshit. Mustang, Hawkeye, Hughes, even their whole little team/crew, their suspicions are okay as long as they don't even give the slightest hint beyond their obvious concern for their youngest colleagues' health, they could probably be trusted with the truth, whatever it is, if they can keep any hint from getting out beyond them. But too many others are involved in a situation like that to risk it in the moment.

On a lighter note, if the siblings are stuck in-hospital for long, how hard would it be for Mustang's minions to find an opportunity to drop in to check on them? Lots and lots of work to worry about with the Incident, especially since they were the team that counts as the First Responders, effectively, as they showed up on their own just in time to start cleaning up and triage the Elrics. Because as I mentioned above, unconscious in a hospital bed is one way that is highly likely to reveal Ed is a female unless there's like a mountain of heavy blankets atop her AND they manage to keep all of the medical staff from mentioning anything. If anything, even an awake-and-obfuscating Ed will get caught out before escaping the nurses/doctors because of one of them loudly scolding her about trying to escape or arguing about meals (ew, milk!) (Or, "Oi, I am an extremely Active Human, I need more calories than that!" vs. Assumptions About Teenaged Girl Diets/Hospital Food). Why would it occur to medics that the clearly female patient isn't /known/ as such unless someone tells them and can Order them to maintain the fiction? Al might be her literal Next Of Kin but he's also even younger than she is, and I wouldn't be surprised if most hospital staff would hesitate at listening to either of them as credible experts or legitimate-adults-who-can-decide-things.
So yeah, unless Mustang/Hawkeye/Hughes think to Do Something on purpose fairly quickly to help keep the secret, Ed is probably going to come out of this with at least a fair number of people at work now aware that she is Not A Boy. I can see lots of ways those three could Help, especially once Al wakes up if Ed doesn't as fast, from charming the medics who'd have discovered already into keeping it quite for the kid's sake with some explanations as to the "why", to doctoring the records being sent up to the General and/or anyone else ever so slightly, to smuggling in some useful bits and bobs to make it less conspicuous to anyone dropping in, like the General will probably want to once they wake up given how big a clusterf*** that turned into.
I wonder if they could collectively charm the General into quietly Not Changing The Records, meaning he wouldn't have Fullmetal's file updated and corrected to reflect her actual name and gender just yet? They could even try to include the "Undercover" card, though that one might not keep him from passing the knowledge up the chain of command under Security Measures so the Fuhrer could still find out then. An "official" record, the one that says Fullmetal is Edward, and a Classified "accurate/actual" one, for Edna, kept under lock and key and need to know at least for now.

I'm also wondering if Ed's leg is still attached, given that Angel had an actual leg there for a bit, the contrast clearly did some damage during the shift. Also, I'm fairly sure this was one of the fights that trashed Edward's automail in canon, though that happened kind of a lot so... Still, different details and handling, similar results in some ways, is kind of a neat parallel.
At least the blatant disaster of a scene would explain something being wrong there, if not exactly fitting with the forensics of how it came off should anyone actually be able to tell. For Ed's sake, I hope it may be damaged but is still mostly attached, because otherwise it probably would have been ripped out at the port itself given the cause and that would mean months of healing, Again, unless Al learned enough healing to shorten things without interfering with whatever is needed for the port-surgery-connection thing. Oh, also blood loss, but given both siblings interest in different branches of healing and Al being awake enough to recognize their friends before passing out with Mustang and co. being there to guard them, something having happened to stop or stall that already would be believable.

Seriously, those homonculi are such a PAIN. Royal, Queen-Bee pain in the ASS.

This story is really interesting to me, because Ed isn't such an arrogant hothead, Alphonse isn't under a delayed death sentence, and the homunculi are entirely unaware of how screwed they are.

Not sure if I'm interpreting this correctly, because it hasn't been outright stated one way or the other before, but I am pretty sure BOTH siblings should be capable of using alchemy by simply slapping their hands together.

Edward already had the knowledge when losing his leg, but pulling his brother into a 'body' required a stabilizing element(blood). Alphonse was capable of it after getting his soul pulled back out. Having 'Angel' trade something instead, no matter what point, would not change this fact. The power was obtained the moment their circle went nuts and started tearing body parts from them.

(Or I missed something massive)

In any case, I am enjoying the story itself.
