I really liked the story, any story where Dean is psychologically tortured via his brother's pain is ace in my opinion. I love hurt Sam and I love protective Dean and I love Sam being rescued, this was awesome!
On a side note, the doctor saying silent is pretty standard. In cases of suspected abuse, all people in the medical field are trained to stay silent for a few minutes. I had pneumonia as a baby and it caused a bone infection in my arm, my mother suspected something was wrong when I'd cry whenever she picked me up under my arms. My father said he had been dressing me the day before and thought he might have hurt my arm by accident. So my mother took me to the hospital and the doctor had asked, "Is there any way she could have . . . fallen?" So she just told them the same thing, that my dad was dressing me and he said he thought he might have hurt my arm. And the doctor sat there, and sat there, and sat there before he said anything else. Two other people did the same thing after asking if I could have fallen. Of course, since I wasn't being abused, my mother just sat there too, silently. It's a a way to bring about a confession or a different story if you're actually hurting your child, if you start changing your story, then they can investigate further.
